Extra billing paying extra money over the money of the service for example a service which costs 100 I pay 120 $ this 20 is the extra billing
User charge
Paying a covered service ,
Covered service means coverd by insurance so we should not pay for it
That what I got
So in question will mention not covered by plan to choose user charge?

Say an surgery X is by 1000 dollars in the public insurance plan. You did this surgery in a hospital and the hospital raise the bill by 1100 dollars. Now this billing is called Extra-billing.
If the patient to do this surgery any amount of money such as any money to get this service earlier, this is called user fees ( any extra money rather than extra-billing).
